賁門炎診斷及常見病因分析

目的 探討胃賁門部炎癥內(nèi)鏡下病理特征并分析常見病因。方法 回顧性分析

我院2016年2月～2018年2月賁門炎患者內(nèi)鏡及病理資料106例，分析內(nèi)鏡下

表現(xiàn)與病理結(jié)果，對賁門炎常見病因進(jìn)行分析。結(jié)果 胃鏡下表現(xiàn)粘膜慢性炎

78.13%，慢性炎伴上皮非典型增生21.88%，高于病理結(jié)果中賁門癌的39.02%，

慢性炎癥41.46%。35例賁門粘膜糜爛中，慢性炎癥62.86%，賁門癌20.00%，

慢性炎伴上皮非典型增生17.14%。賁門炎合并糜爛性食管炎51.85%，導(dǎo)致食管

粘膜損傷的胃食管反流病60.18%。結(jié)論 賁門炎充血水腫一般為慢性炎癥，合并

糜爛粗糙者一定要取活檢；賁門炎多為胃食管反流引起。

Abstract：Objective To investigate the endoscopic pathological features of gastric

cardia and its common s The endoscopic and pathological data of 106

cas patients with cardia from February 2016 to February 2018 were retrospectively

endoscopic findings and pathological findings were analyzed to analyze

the common caus of s Gastric microscopic mucosal chronic

inflammation was 78.13%，chronic inflammation with epithelial atypical hyperplasia

was 21.88%，higher than pathological results，39.02% of gastric cardia cancer and

41.46% of chronic 35 cas of gastric mucosal erosion，

chronic inflammation was 62.86%，cardiac cancer was 20.00%，and chronic

inflammation was associated with epithelial atypical hyperplasia 17.14%.Cardiac

inflammation combined with erosive esophagitis 51.85%，causing esophageal

mucosal injury of gastroesophageal reflux dia 60.18%.Conclusion Cardiac

inflammation， congestion and edema is generally chronic inflammation， combined

with rough erosion must take biopsy；cardia inflammation is mostly caud by

gastroesophageal reflux.

胃賁門部由于其特殊解剖位置、生理功能及相關(guān)病理生理機(jī)制，成為胃食管

反流病、胃食管結(jié)合部腫瘤等疾病的好發(fā)部位[1]。賁門炎是慢性胃炎的一種類

型，與胃食管反流病、Barrett食管等疾病存在一定關(guān)系[2]。內(nèi)鏡下賁門部有充

血水腫、糜爛等，內(nèi)鏡醫(yī)生會(huì)取活組織檢查，但關(guān)于是否一定要取活檢及活檢結(jié)

果分析國內(nèi)報(bào)告不多。本文采用回顧性分析我院胃鏡室2016年2月～2018年2

月使用Olympus260型胃鏡鏡下診斷賁門炎患者106例，與病理活檢結(jié)果對比分

析，總結(jié)鏡下賁門充血水腫等表現(xiàn)與病理結(jié)果比較，指導(dǎo)內(nèi)鏡醫(yī)生對賁門部病變

認(rèn)識(shí)，提高確診率；并分析導(dǎo)致賁門炎的常見疾病，提醒臨床醫(yī)生積極治療導(dǎo)致

賁門炎發(fā)生的常見疾病，預(yù)防賁門炎的發(fā)生。

1 資料與方法

1.1資料來源 選取武漢大學(xué)醫(yī)院2016年2月～2018年2月經(jīng)內(nèi)鏡檢查診斷
